What is Chronic Care Management?

Are you worried about your health?

Do you feel like your care requires more organization?

Would you like to have your own personal medical assistant who has direct access to Dr. Pulido who can help with medications, referrals, appointments, medical advice, and more?

Emed is now providing Chronic Care Management to all its patients who have Medicare or a Medicare Advantage plan and whose insurance company will pay for it. (If it is unclear if your insurance covers this program, please call them and ask if CCM is covered.)

-A monthly phone call from Dr. Pulido’s personal assistant whose sole purpose is to help organize your healthcare

-Help With Medical Advice
-Help with Medication prior authorizations

Requirements

-Must be an Emed Primary Care Patient
-Must have Medicare, Medicare Advantage, or an insurance that pays for Chronic Care management
-Must have a signed Chronic Care Management Consent Form (below)
